During your period you are plagued by excruciating pain that goes far beyond the usual menstrual symptoms? Menstrual cramps limit you in your everyday life and sometimes they are so overwhelming that you can hardly move or work. One cause has not yet been found - you have to cope with the gruelling pain that can extend into your leg and lower back. Month in and month out. But that is not something you really want.
With endometriosis, cells of the uterine lining - the endometrium - can spread outside the uterine cavity. They can settle on the peritoneum of the pelvis, the ovaries, the bladder or the intestines, sometimes even on the lungs or the kidneys. Since the endometriosis lesions are subject to cyclic hormonal changes, they grow and release chemical substances during menstruation, which trigger pain in the lower abdomen during the period. Until the opposite is proven, lower abdominal pain during menstruation indicates endometriosis. In many women, the pain extends to the leg or the lower back. Sometimes it can even feel like a lumbago or irritation of the sciatic nerve.
Many women have undergone numerous changes of doctor and countless examinations, but there is no evidence of a specific clinical condition that explains the severe pain experienced during the period. Often many painful years pass until the cause becomes clear. The possibility of endometriosis is often not taken into consideration because the pain symptoms cannot always be clearly identified. However, because endometriosis can lead to infertility in the worst case, treatment is especially important.
Endometriosis cannot be completely detected even with the most modern ultrasound equipment. Only a laparoscopy allows a reliable and complete diagnosis. Through a small incision in the navel, a laparoscope is inserted - an instrument with a tiny camera and light - that makes the endometriosis lesions visible. If cell accumulations are found, they can be removed in the same procedure using fine surgical instruments that are inserted through further incisions.
Endometriosis can affect women throughout their fertile life, from the beginning of the first menstrual period to before menopause. Most patients find the diagnosis a relief - they finally have certainty and proof that the pain is not imaginary. Since endometriosis is a benign but chronic disease, repeated interventions may be necessary.
Endometriosis can not only affect the tissues and organs in the pelvic area, the cells can also settle on the sciatic nerve and on any of the nerves in the pelvis. These include the obturatorius nerve, a nerve of the lumbar plexus, and the femoral nerves (thigh nerves). In this form of endometriosis, surgical therapy is necessary to avoid irreversible nerve damage, which leads to motor impairments and particularly affects the lifting and lowering of the foot.
As in the case of endometriosis of the organs, the cell tissue is also removed by laparoscopic surgery if the sciatic nerve is affected.
