Special surgical gynecology for complex procedures 

The advanced training in "special operative gynaecology" goes far beyond the usual operative training of the gynecologist. It includes the acquisition of the special skills required to carry out complex gynaecological interventions. These include above all the surgical treatment of genital cancers and deep infiltrating endometriosis, as well as the treatment of women's prolapse and incontinence problems - diseases that are extremely distressing for those affected. In combination with laparoscopy, these procedures are possible on a minimally invasive basis. The excellent treatment results help patients to achieve a noticeably better quality of life.

While Prof. Possover enjoys a worldwide reputation today as a pioneer of Neuropelveology, about 20 years ago he was known as a pioneer of laparoscopic radical pelvic surgery. Since Prof. Possover began his medical studies at the age of 15, he set this evolution in motion at a relatively young age.

Training in "special operative gynaecology" was already being offered at the end of the 1990s. At that time, this training was mainly concerned with the surgical treatment of genital cancer by abdominal incision. Since cancer can also affect neighbouring organs, the oncological surgeons of the time also learned to operate on neighbouring organs such as the intestine, bladder and ureter. The surgeons also made use of this ability in the surgery of deep infiltrating endometriosis - a severe form of endometriosis which, like cancer, can also affect the adjacent organs of the bladder, intestine, ureter or pelvic nerves. For this reason, the transfer of knowledge was aimed at cancer and endometriosis surgery.

Prof. Possover was the first to introduce laparoscopy in this surgical field. In Jena/Germany he introduced the completely new vaginal surgery of uterine cancer (named the Schauta operation) and combined this intervention with the removal of the pelvic and abdominal lymph nodes by means of laparoscopy - the "laparoscopic lymphonodectomy". The University of Jena was the epicenter of laparoscopic oncosurgery in Europe. All oncologists working laparoscopically in gynaecology today are thus directly or indirectly students of Prof. Possover, who passed on his knowledge in order to make the minimally invasive techniques accessible to as many patients as possible.

In 1996 Prof. Possover also performed the first laparoscopic bowel resection for the treatment of rectal endometriosis in Germany. This was followed by the first bladder and ureter resections and, about 10 years ago, laparoscopic intervention for the treatment of endometriosis of the sciatic nerve - an area in which Prof. Possover is regarded as the world's leading expert.

Prof. Possover has thus not only set milestones in surgical gynaecology, but over the last 20 years has also overseen a continuous development process that has brought laparoscopic surgery far ahead in gynecology. Neuropelveology and the associated findings regarding the pelvic nerves have also revolutionized this field of surgery.
